HBNC CVS 1Community Visitors Scheme

The Community Visitors Scheme provides trained and friendly volunteer visitors to older people in our community at risk of social isolation.  Community Visitors provide one-to-one visits once a week or once per fortnight and provide friendship and companionship.

The Scheme is funded by the federal Department of Health.  The CVS service is free to residents of aged care facilities in the Wide Bay, and for recipients of Commonwealth Care Packages living at home in the Fraser Coast.

Community Visitors share their time and interests and visits are relaxed and social, where you can chat, reminisce, listen to music, read the newspaper, play board games or other hobbies and fun activities.

Referral to the CVS:

Once we have received your referral, the CVS Project Officer will arrange to visit you in your home.  In this meeting, the Project Officer will have a friendly chat and get to know you, fill in a registration form and complete a Home Environment Assessment and obtain consent for CVS service provision.  Once registered, the Project Officer will match and introduce a community visitor to begin social visits.  The Project Officer then stays in regular contact with you both to ensure everyone enjoys their visits.
